Six Central Massachusetts hospitals have been selected to participate in an affordable care act program aimed at improving patient transitions between health care settings and reducing readmissions, the Massachusetts Hospital Association announced.
Clinton Hospital, HealthAlliance Hospital, Marlborough Hospital, MetroWest Medical Center, Saint Vincent Hospital and UMass Memorial Medical Center will partner with Worcester-based Elder Services and Bay Path Elder Services in the program. Wing Memorial Hospital in Palmer, a UMass hospital, will also participate.
