Fallon expands N.Y. managed care program

(Updated Feb. 24 at 2 p.m.) Fallon Health of Worcester has expanded the long-term-care program in Western New York it offers in conjunction with a residential community there.

Fallon Health Weinberg in Amherst, N.Y., said Wednesday it is accepting eligible individuals in Niagara County. It has already been accepting individuals in Erie County. The program is open to people 21 and older who require long-term care for more than 120 days and who are eligible for Medicaid.

Fallon Health and Weinberg Campus, both nonprofits, began their joint venture last fall.

Fallon Health Weinberg MLTC (Managed Long Term Care) was formerly known as the Total Aging in Place Program (TAIPP). Through its services include home care, non-emergency transportation and adult day programs, the program helps people with chronic illnesses or long-term disabilities live in their own homes and communities.

“Our goal is really quite simple: We help members live the kind of lives they want to live,” Fallon President and CEO Patrick Hughes said in a statement. “With all our plan offerings, Fallon Health Weinberg offers a simple and coordinated approach to health challenges that individuals may have.”
