Seven Hills tapped for Medicaid management

Courtesy Seven Hills Family Services will lead five community partners in coordinating care for 4,500 Medicaid patients in Central, Northeastern and Southeastern Massachusetts.

Worcester nonprofit Seven Hills Family Services, an affiliate of the Seven Hills Foundation, has been selected by the Executive Office of Health and Humans Services to lead agencies across Central and Eastern Massachusetts in coordinating care for 4,500 Medicaid patients, the nonprofit said in a statement last week.

Doing business as the Massachusetts Care Coordination Network (MCCN), Seven Hills and five community partner agencies will provide coordination for MassHealth patients with long-term care and support needs, Seven Hills said. The partner agencies are designated Long-Term Services and Supports partners. The MCCN will operate in the Central, Southeastern and Northeastern regions of Massachusetts.

Seven Hills was selected under the Long Term Services and Supports Community Partner program, a five-year program designed by the state in response to a national redesign to the federal Medicaid program.
