Tufts Health Plan expanding Worcester partnership to Middlesex and Suffolk counties

After launching collaboration in Worcester County, Tufts Health Plan, a nonprofit health insurer headquartered in Watertown, is expanding its partnership with Brooklyn-based community healthcare provider Cityblock Health to benefit Tufts Health Unify members in Middlesex and Suffolk Counties, according to a Wednesday press release.

Cityblock first began its work in Massachusetts in March 2020, collaborating with Tufts Health Plan to provide care to members in Worcester County, according to the announcement. Since then, the partnership has served nearly 1,800 low-income individuals living with disabilities.

The expansion to Middlesex and Suffolk Counties will double Cityblock’s presence in the state to provide care for 3,000 members, according to the release.

“The pilot with Cityblock showed crucial evidence of the strengths of the model, with more than 75% of our eligible Unify members being engaged by their care teams,” said Jean Yang, president of Tufts Health Plan’s Public Plans division, in a statement. “We’re thrilled to expand the reach of these services to members in Middlesex and Suffolk Counties, and seek new and innovative ways to combat the social determinants of health.”

Cityblock Health uses a community-based care model to connect Tufts Health Unify members to medical, behavioral, and social health care. The model addresses social determinants of health, such as housing, employment, transportation, food, and childcare, at no extra cost to the member, according to the press release.

Tufts Health Public Plans Inc. is the second largest health insurer in the state, with almost 492,000 members and a 22.3% share of the Massachusetts market.
