You are invited to attend a Ribbon Cutting Ceremony and Grand Opening celebration at Form First Physical Therapy on Monday, November 21, from 3:00pm-6:00pm. The new office is located at 48 Main Street (Rte. 131) in Sturbridge.
Form First Physical Therapy provides personalized treatment options for all of their patients. Whether you are returning to sports, recovering from surgery, suffering with chronic pain or seeking to improve mobility, Form First Doctors take the time to address your needs and work with you to establish an individualized plan of care every time.

The Ribbon Cutting ceremony will be presented by the Chamber of Central Mass South at 3:00pm with local dignitaries and the snipping of the official red ribbon. After the ceremony, all are invited to meet the owners, Nick Murray PT DPT and Christina Murray PT DPT, enjoy light appetizers and a glass of wine.
Guests will have the opportunity to “Ask a PT” and get a professional opinion on any injury or pain they might have, as well as receive a free fall risk/balance assessment. Form First will also offer raffles for local restaurant gift cards at their Grand Opening!
Everyone is welcome to attend. There is no cost and no need to RSVP. Just come on by and join in as the Chamber and our guests provide a warm Sturbridge welcome and best wishes to our newest members!
